摘要
目的观察和评估伴近视的知识分子白内障患者超声乳化植入单焦人工晶状体术后保持良好视觉屈光状态的临床效果。方法对40例知识分子白内障伴近视患者进行超声乳化人工晶状体植入,术前预留人工晶状体度数,使其术后成为近视屈光状态;术后视远佩戴近视眼镜,视近不戴眼镜。定期随访远、近视力等,并对生活影响满意度进行问卷调查。结果术后患者矫正视力≥0.6,近视力优于Jaeger2,术前理想的预留屈光度为-3.5^-4.0D,偏低者近视力差,偏高者远视力差。结论知识分子患者白内障术后预留一定近视度数符合患者的生活习惯,减少近距离的调节,术后佩带眼镜可以完全矫正术后的少量散光,使患者对医疗手段满意。如果术后能佩带具有一定防护功能的眼镜,对视网膜尤其黄斑起到一定防护作用。
Objective To observe and evaluate the clinical therapeutic effect of intellectual patients with myopia and cataract, who were treated with phacoemulsification and monofocal intraocular lens implantation, and designed mild myopic status after the surgery. Methods Forty intellectual patients with myopia and cataract were treated with phacoemulsification and monofocal intraocular lens implantation. Intraocular lens power was designed presurgically to make them in mild myoptic status. After surgery, the patients wore myoptic spectacles for watching distant objects and wore no spectacles for watching near objects. Distant and near best corrected visual acuity (BCAV), and standard questionnaire of visual life quality were performed in the regular postsurgical follow-up. Results After surgery, the patients' distant BCVA was equal to or better than 0.6, near BCAV was equal to or better than 2 of Jaeger Standard Print. The optimal presetted myopic status of IOL power design was - 3.5 to - 4.0 Dipter. The lower the diopter, the poorer the near visual acuity of naked eyes was; The higher the diopter, the poorer the distant visual acuity of naked eyes was,Conclusion The presetted mild myopic status of intellectual patients after cataract surgery is accorded with their daily life habits by reducing near distance accommodation. The residual astigmatism after surgery can be completely corrected by postoperative spectacles, which satisfies the patients.
