慢性乙型肝炎和肝硬化患者结缔组织生长因子的肝内表达

Study on intrahepatic express of connective tissue growth factor in chronic hepatitis B and cirrhosis

摘要 目的探讨结缔组织生长因子(CTGF)的肝内表达与肝硬化的关系。方法对93例慢性乙型肝炎和肝硬化患者的肝组织活检样本,采用免疫组化方法进行CTGF肝内表达研究,结合HE染色和网状纤维染色,分析比较慢性乙型肝炎和肝硬化的CTGF肝内表达及表达的细胞定位。结果肝硬化患者CTGF的表达强度明显高于慢性乙型肝炎患者(P<0.01),肝纤维化程度越高,CTGF表达越强烈。CTGF的表达与慢性乙型肝炎的炎性活动程度明显相关(P<0.05)。CTGF的表达主要集中于汇管区和纤维化区,肝星形细胞、成纤维细胞和窦状隙内皮细胞染色常呈阳性。结论CTGF的表达与肝纤维化及肝硬化程度密切相关,肝星形细胞、成纤维细胞和窦状隙内皮细胞是肝内CTGF的重要来源。 Objective In order to explore the relationship between intrahepatic express of connective tissue growth factor(CTGF) and liver fibrosis or cirrhosis due to hepatitis B virus infection. Methods 93 samples of liver biopsies of chronic hepatitis B (CHB) and cirrhosis was enrolled in the study. Intrahepafic express of CTGFF was assayed by immunohistocbemical staining. Results Express of CTGF in cirrhosis was significantly stronger than in CHB ( P 〈 0.01 ), and was correlated with inflammation of hepatic tissue( P 〈 0.05). Express of CTGF was observed in portal tracts and fibrous septa predominantly in the fibrogenesis areas. The positive staining was observed in hepatic stellate cells, sinusoidal endothelial cells and fibroblasts. Conclusions Intrahepatic express of CTGF has an important rule in pathogenesis of fibrosis or cirrhosis. Hepatic stellate cells, sinusoidal endothelial cells and fibroblasts seem to be the major cellular sources of CTGF in the liver.
