多肿瘤标志物蛋白芯片检测系统对胰腺癌的诊断价值评价 被引量:4

摘要 目的:研究多肿瘤标志物蛋白芯片诊断系统对胰腺癌的诊断价值。方法:用多肿瘤标志物蛋白芯片诊断系统测定分析43例胰腺癌患者,54例胰腺良性疾病患者和30例健康体检者血清的12种常见肿瘤标志物:糖原199(CA199)、神经原特异性烯醇化酶(NSE)、癌胚抗原(CEA)、糖原242(CA242)、铁蛋白(FER)、β-人绒毛膜促性腺激素(β-HCG)、甲胎蛋白(AFP)、游离前列腺特异抗原(f-PSA)、前列腺特异抗原(PSA)、糖原125(CA125)、人生长激素(HGH)、糖原153(CA153)。主要从真实性、可靠性两方面评价其诊断效能。结果:联合CA199、CA242、CEA诊断胰腺癌的真实性评价指标:灵敏度86·0%、特异度66·7%、误诊率(α)33·3%、漏诊率(β)14·0%、阳性似然比2·58、阴性似然比0·21、诊断指数153%、Youden指数0·53、粗符合率75·3%;可靠性评价指标(一致性相关系数):CA199(0·846,P<0·01)、NSE(0·858,P<0·01)、CEA(0·906,P<0·01)、CA242(0·864,P<0·01)、FER(0·913,P<0·01)、β-HCG(0·881,P<0·01)、AFP(0·894,P<0·01)、f-PSA(0·836,P<0·01)、PSA(0·875,P<0·01)、CA125(0·866,P<0·01)、HGH(0·887,P<0·01)、CA153(0·875,P<0·01)。结论:多肿瘤标志物蛋白芯片诊断系统对胰腺癌有一定的辅助诊断价值,联合CA199、CA242、CEA可显著提高胰腺癌诊断的灵敏度。 Objective: To investigate the application of Multi-tumor markers Protein Chip Diagnostic System in the diagnostic of pancreatic cancer. Methods: Using the Multi-tumor markers Protein Chip Diagnostie System to determine and analyze the concentration values of 12 tumor markers (CA199, NSE, CEA, CA242, FER, β-HCG, AFP, f-PSA, PSA, CA125, HGH, CA153) in the serum of 43 patients with pancreatic cancer, 54 benign pancreatic disease and 30 normal persons.By analyzing the validity and the diagnostic potency of the Multi-tumor markers Protein Chip Diagnostic System was evaluated. Results: The validity of this system in the diagnosis of the pancreatic cancer with combination of CA199, CA242, and CEA was evaluatid according the following criteria: sensitivity (86.0%), specificity (66.7%), a (33.3%), 13 (14.0%), positive likelihood ratio (2.58), negative likekihood ratio (0.21), diagnostic index ( 153% ), Youden' s index (0.53), and crude accuracy (75.3%) ; the criteriafor reliability ( concordance correlation coefficient) were. CA199 (0.846, P 〈 0.01 ), NSE (0.858, P 〈 0.01 ), CEA (0.906, P〈0.01), CA242 (0.864, P〈0.01), FER (0.913, P〈0.01), β-HCG (0.881, P〈0.01), AFP (0.894, P〈0.01), f-PSA (0.836, P〈0.01), PSA (0.875, P〈0.01), CA125 (0.866, P〈0.01), HGH (0.887, P〈 0.01 ), CA153 ( 0. 875, P 〈 0.01 ) . Conclusions: The Multi-tumor marjers Protein Chip Diagnosde System had valid valve for its clinical appucation in the diagnosis of pancreatic cancer.The sensitivity could be increased with the combination of CA199, CA242, and CEA.
