射频消融治疗不能切除的胰体尾癌 被引量:8

摘要 目的评估射频消融治疗不能切除的胰体尾癌的临床价值。方法对手术不能切除的10例胰体尾癌行术中射频热疗,观察并发症、近期疗效及生存情况。结果部分病例活检病理学报告为凝固坏死改变,术后CT为高密度改变,B超为高回声改变。所有病例均未并发胰瘘,术后腹痛明显缓解,术前8例CA19-9增高的患者术后明显下降。平均随访15个月,2例死亡,8例带瘤健康存活,生存质量尚可。结论RFA治疗不能切除的胰体尾肿瘤是安全的。 Objective To evaluate the clinical value of radiofrequency ablation (RFA) in treating unresectable carcinoma of the body and tail of the pancreas. Methods Intraoperative RFA was performed in 10 patients with unresectable carcinoma of the body and tail of the pancreas, and the complications, short-term curative effect and patients' survival were observed. Results Coagulative necrosis was observed in HE stained slices of RFA lesions in a few cases. The CT scanning after ablation showed hyperdensity region, and the ultrasonic image revealed high echo region. Epigastric pain was relieved in all patients after operation, and there was no occurrence of pancreatic fistula. The preoperative high serum levels of CA19-9 in 8 cases reduced markedly. Median duration of follow-up was 15 months. 2 patients died and the other 8 survived. Conclusions RFA is effective and safe for the treatment of unresectable carcinoma of the body and tail of the pancreas.
