家用无创机械通气治疗慢性阻塞性肺疾病的护理

Nursing Care of Chronic Obstructive Pulmonary Disease Using Domestic Minimal Invasive Mechanical Ventilation

目的探讨慢性阻塞性肺疾病患者出院后在家庭继续使用无创机械通气的护理。方法选择2004年5月～2005年3月在我院住院并接受无创机械通气治疗的慢性阻塞性肺疾病患者20例，出院后继续在家中采用家庭型呼吸机治疗的患者10例为实验组，出院后用氧气治疗的患者10例为对照组，1个月后观察两组患者肺功能指标。结果出院后1个月两组患者的肺功能指标比较差异有显著性（P〈0．05）。结论对患有慢性阻塞性肺疾病患者出院后若能继续使用家庭型无创机械通气进行治疗并采取积极有效的护理措施将有效改善患者残余肺组织的通气功能。有利于全面提高生活质量。

Objective To probe the effect of using damestic minimal invasive mechanical ventilation with it＇s nursing care at home in chronic obstructive pulmonary disease（COPD）patients after discharge from the hospital. Method Ten cases of COPD were recruited as the experimental group, all of whom had received minimal invasive mechanical ventilation treatment during their stay in our hospital from May 2004 to March 2005, and kept up the treatment with the machine at home after discharge. Another 10 cases with the same disease status who had received similar treatment in the hospital, but changed to oxygen inhalation treatment at home, served as the control group. Blood gas analyses of both groups were compared. Result Both the experimental and the reference group showed remarkable improvement in blood gas test result after minimal invasive mechanical ventilation treatment during their stay in hospital, with no significant differences between the two groups （ P 〉 0. 05）. One month later after they left hospital, the two groups＇ blood gas test indices were compared and notable differences were found （ P 〈 0.05）. Conclusion The ventilatory function of the remnant lung tissues would be improved, the frequency of hospitalization reduced and the life quality improved if the patients with COPD keep up the minimal invasive mechanical ventilation treatment at home with effective nursing measures.
