摘要
目的:探讨自发性肾破裂的病因、临床诊断及治疗。方法:回顾分析5例患者临床资料,结合文献进行讨论。结果:4例患者急性发病,B超及CT证实有肾周出血,同时发现肾脏肿瘤,肿瘤内可见脂肪密度,最终行开放手术治疗。1例症状不典型,辅助检查证实左肾被膜下积液,逆行造影检查见左侧肾盂中上盏处有少量造影剂溢出,行患肾双J管置入术,4周后复查被膜下积液完全吸收。结论:肾血管平滑肌脂肪瘤是引起自发性肾破裂最常见的病因,B超及CT有重要的诊断价值,其治疗主要根据原发病的诊断及出血情况。
Objective： To investigate the cause, diagnosis and management in patients with spontaneous kidney rupture. Methods： The clinical data of 5 cases with spontaneous kidney rupture were retrospectively analyzed. Results： B-ultrasonography and CT scanning showed perirenal hemorrhage and tumor in the kidney in g cases, of whom 2 underwent open surgery, and showed subcapsular hematoma in 1 case who was performed cystoscopy and retro urography which showed extravasation of contrast medium in subcapsular regions, suggesting pelvicalyceal rupture, and was inserted a double-J stent. Conclusion： Angiomyolipoma of kidney is the main cause for spontaneous kidney rupture. B-uhrasonography and CT scanning have important value for diagnosis. The management depends on the diagnosis of original diseases and the condition of bleeding.
