斯奇康局部封闭治疗皮肤结核疗效观察

Local hypodermic Siqikang(BCG-PSN) injection for treatment of tuberculosis cutis

目的提高皮肤结核的疗效。方法在应用短程督导化疗(DOTS)的基础上,加用斯奇康(BCG-PSN)注射液局部封闭疗法,并与既往单纯应用短程督导化疗组作对照。结果疗后1个月两组治愈率分别为51.61、21.88%(χ2=6.01,P<0.01);总有效率分别为77.42、46.88%(χ2=6.23,P<0.01)。疗后2个月时,两组治愈率分别为90.32、59.38%(χ2=7.96,P<0.01);总有效率分别为80.65、75.00(χ2=4.45,P<0.05)。疗后3个月时,两组治愈率分别为93.55、71.88%(χ2=8.00,P<0.01);总有效率两组无差异(P>0.05)。皮损面积:疗后第1、2、3个月两组相比(均P<0.05)。疗程结束时疗效指数:斯奇康组99.86±0.30,对照组98.10±4.47(t=2.29,P<0.05)。临床症状总积分:疗后1个月两组分别为1.19±0.83、1.75±0.76(t=2.95,P<0.05);疗后2个月两组分别为0.45±0.51、1.06±0.72(t=4.07,P<0.05);疗后3个月斯奇康组0.10±0.30,对照组0.50±0.67(t=3.64,P<0.01)。结论在DOTS基础上,联合应用双相免疫调节剂——斯奇康局部封闭疗法治疗皮肤结核,可明显增强患者的免疫功能,缩短病程,提高疗效。

Objective To study the efficacy of Bacillus Calmette-Guerin Polysarides-Nuclear Aeid（BCG-PSN） in the treatment of tuberculosis eutis. Methods 63 patients were divided into two groups. Both groups were treated by Direct Oversee Treat Short （DOTS）. In addition, the treatment group was subcutaneously injected BCG-PSN with the dose of 0.5ml/cm^2 once per week and control group was only given DOTS. Results The effective rate was 77.42% in the treatment group and 46.88% in the control group in the first month（x^2= 6.23,P〈0.01）. After the second and third month, a notable difference was noticed between the two groups（P〈0.01） in terms of the cure rate, skin lesion dimensions, clinical symptoms and cure indexes. Conclusion DOTS ＋ BCG-PSN to treat tuberculosis eutis will substantially enhance immunological function, promote recovery and raise efficacy.
