感染性心内膜炎并发腹腔干假性动脉瘤破裂致死亡一例

摘要
患者，女，18岁。乏力5个月，心慌气短1个月。于5个月前出现乏力，渐加重，3个月前在当地医院诊断为肾小球肾炎。1个月前自诉心慌气短，发热两天（体温达39℃）。心脏彩色超声心动图（UCG）示：二尖瓣前叶赘生物，二尖瓣反流。近1周心悸、乏力加重，尿量减少。入院查体：血压100／80mmHg（1mmHg=0．133kPa），慢性病容，心率105次／分，节律整齐，心尖部可闻及收缩期杂音，腹软、无压痛及反跳痛。UCG示：二尖瓣叶损害，后叶脱垂，前叶见2mm×3mm赘生物。心电图：窦性心律，左心室高电压。腹部超声；腹主动脉上段动脉瘤。
