摘要
目的探讨2型糖尿病患者血、尿转化生长因子(TGF)-β1、尿Ⅳ型胶原与糖尿病早期肾病的关系。方法149例2型糖尿病患者根据24h尿白蛋白排泄率(UAER)分为正常蛋白尿组(UAER〈30mg/24h)和微量白蛋白尿组(UAER30~300mg/24h)。结果微量白蛋白尿组的24h尿TGF-β1、尿Ⅳ型胶原和血TGF-β1显著高于正常蛋白尿组(P值均〈0.01)。24h尿TGF-β1、尿Ⅳ型胶原和血TGF-β1均与UAER呈正相关(P〈0.01)。分别以UAER、尿TGF-β1、尿Ⅳ型胶原、血TGF-β1为应变量(Y),以病程、年龄、体重指数、腰臀比、收缩压、舒张压、空腹血糖、三酰甘油、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、C肽、糖化血红蛋白(HbA1c)为自变量(X1、X2、X1…X12)作多元逐步线性回归分析。LDL、HbA1c进入UAER的回归方程(Y=0.959+0.532X9+0.188X12);HD12);HbA1c进入24h尿TGF-β1的回归方程(Y=3.263-0.47X10+0.212X12),HbA1c进入24h尿Ⅳ型胶原的回归方程(Y=4.032+0.106X12);LDL、HbA1c进入血TGF-β1的回归方程(Y=14.51+1.413X1+7.622X12)。结论24h尿TGF-β1、尿Ⅳ型胶原和血TGF-β1可作为糖尿病早期肾损害较敏感的指标。
Objective To appraise the relevance of serum and urinary transforming growth factor(TGF)-β1 and urinary type Ⅳ collagen(Ⅳ-C) with early diabetic nephropathy in type-2 diabetic patients. Methods 149 cases of typ-2 diabetes were divided into normoalbuminuria group and microalbuminuria group, according to the following items: urine albumin excretion rate(UAER). UAER, TGF β1 , Ⅳ-C, blood TGF-β1 , blood glucose, blood lipid, C-peptide, blood creatinine(Cr), urea nltrogen(BUN), glycosylated hemoglobin ( HbA1 c), were measured. Results 24 h urinary TGF- β1 , Ⅳ C and serum TGF-β1 in the microalbumlnuria group were markedly higher than those in normoalbuminuria group(P〈0.01) ,24 h urinary TGF-β1 , Ⅳ-C and serum TGF-β1 were also positively correlated with UAER(P〈0.01). Conelusions 24 h urinary TGF-β1 , Ⅳ-C and serum TGF-β1 seem to be the early markers of diabetic nephropathy. (Shanghai Med J, 2006, 29:110 112)
