非超声乳化小切口与现代囊外白内障摘除术临床比较

Clinical comparison between sutureless small incision extracapsular cataract extraction and modern extracapsular cataract extration

目的比较非超声乳化小切口与现代囊外白内障摘除术对术后视力、角膜散光、角膜水肿的影响。方法将157例(174只眼)白内障患者分为两组,分别施行非超声乳化小切口与现代囊外摘除术,术后观察视力、角膜散光、角膜水肿。结果两组患者术后角膜散光有差异性(P<0.05),术后视力无差异性(P>0.05),对于核硬度Ⅰ～Ⅱ级的患者,两种术式对角膜水肿无影响(P>0.05),对于核硬度Ⅲ～Ⅳ级患者,两种术式对角膜水肿有差异性(P<0.05)。结论对年龄大、晶状体核硬的患者,虽然现代囊外摘除术后散光大一些,但视力不受影响,角膜水肿影响小,故应选用白内障现代囊外摘除术,对于晶状体核小者,选择非超声乳化小切口手术。

Objective To compare the difference of visual acuity, corneal astigmatism and corneal dropsy after surureless small incision extracapsulor catarat extraction and modern extracapsular catarat extration. Methods Sutureless small incision extracapsular cataract extraction and modern extracapsular cataract extration were performed on 174 eyes of 157 cases respectively. Postoperative visual acuity, corneal dropy and corneal astigmatism were compared. Results There was significant difference in postoperative corneal astigmatism （P〈0.05）. There was no significant difference in postoperative visual acuicy （P〉0.05）, but the difference was significant for those whose hardness of nuclear was Ⅲ～Ⅳin postoperative corneal dropsy （P〈 0.05）. While for those hardness of nuclear was Ⅰ～Ⅱ the difference was not significant of corneal dropsy （P〉0.05）. Conclusion Modern extracapsular cataract extration should be chosen for those elder men whose nuclear was hard. Although postoperative corneal astigmatism is a little larger but the visual acuity and corneal dropsy are less influenced, sutureless small incision extracapsular cataract extraction should be chosen for those whose hardless of nuclear.
