鼻内镜下顽固性鼻出血的治疗体会

Experience of treating obstinate epistaxis by rhinoscope

目的:探讨顽固性鼻出血在鼻内镜下的治疗方法。方法:表面麻醉下行鼻内镜检查,查找并明确出血部位,对活动性出血予微波凝固。对粘膜糜烂渗血应用明胶海绵贴附,再用艾氟隆填塞。结果:31例顽固性出血患者,28例一次治愈,其中4例合并有鼻中隔偏曲者经手术后未再继续出血。2例黎特氏区出血者,经再次微波治疗后出血停止,1例血小板减少性紫癫患者因治疗原发病转入他院治疗失访。结论:对于顽固性鼻出血患者,鼻内镜下采用微波,明胶海绵及艾氟隆填塞治疗为一种简单、迅速、痛苦小且疗效好的方法。

Objective To explore the therapy of obstinate epistaxis by rhinoscope. Methods To perform endoscopy after topical anesthesia, locate the bleeding part and solidify it by microwave. To stick gelatin sponge on it and fill Ivalon in it. Results of 31 cases,28 cases were cured, and 4 cases with septal deviation stopped bleeding after operation. 2 cases of Litfle＇s area stopped bleeding after microwave treatment, and one case of thrombocytopenic purpura was not followed.
