脾静脉管径及脾脏厚度与肝组织纤维化程度关系的研究

The relationship between the extent of liver fibrosis and the thickness of spleen and the diameter of splenic vein

目的了解脾静脉管径宽度及脾脏厚度与肝组织纤维化程度的关系。方法 166例慢性病毒性肝炎住院患者在B超引导下行肝活检,肝组织按肝纤维化分级标准分为S_0～S_4。B超测量患者脾静脉管径宽度(SPV)及脾脏厚度(SPT)。结果 S_0至S_4组脾静脉管径宽度及脾脏厚度分别是：S_0为(5．0±0．9)mm和(31．7±6．9)mm,S_1为(5．5±1．0)mm和(33．3±4．3)mm,S_2为(6．2±1．0)mm和(36．2±5．5)mm,S_3为(7．1±1．8)mm和(42．1±13．6)mm,S_4为(8．6±2．8)mm和(47．4±11．6)mm。自S_2到S_4期脾静脉管径宽度逐步增加；S_3、S_4期脾脏明显逐渐增厚。结论脾静脉管径宽度及脾脏厚度对慢性病毒性肝炎患者肝组织纤维化程度有一定的诊断价值。

Objective To study the relationship between the liver fibrosis degree and the width of spleen vein （SPY） and the thickness of spleen（SPT）. Methods Liver specimens from 166 patients with chronic viral hepatitis were divided into 5 groups from SO to S4 according to the liver fibrosis degrees. SPV and SPT were measured by ultrasonic B. Results The results of SPV and SPT of groups S0,S1,S2,S3 and S4 were （5.0 ± 0.9）mm and （31.7 ± 6.9）mm, （5.5 ± 1.0）mm and （33.3 ± 4.3）mm, （6.2 ± 1. 0）mm and （36.2±5.5）mm, （7.1 ± 1.8）mm and （42.1± 13.6）mm, （8.6±2.8）mm and （47.4± 11.6）mm. SPV dilates significantly and gradually with developing liver fibrosis degrees from S2 to S4, SPT increases sigrdficantly and gradually with developing liver fibrosis degrees from S3 to S4 too. Conclusion Observations on SPY and SPT have some certain significance to diagnosis the degree of liver fibrosis in patients with chronic viral hepatitis.
