苈黄导痰汤治疗慢性阻塞性肺疾病急性加重期临床观察

A Clinical Study on Treatment of Chronic Obstructive Pulmonary Disease Exacerbationwith Li Huang Dao Tan Decoction

目的:观察苈黄导痰汤治疗慢性阻塞性肺疾病急性加重期的临床疗效并探讨其作用机制。方法:治疗组给予苈黄导痰汤及常规西药,对照组给予常规西药,观察临床疗效、住院时间、主要症状体征缓解时间及血气分析指标等,两组间进行对照。结果:治疗组临床疗效优于对照组(P<0.05);治疗组住院时间(12.5±5.8)天,对照组(14.7±6.3)天(P<0.05);治疗组各主要症状体征缓解时间短于对照组(P<0.05,P<0.01),PaO2、P(A-a)O2的改善优于对照组(P<0.05);治疗组痰浊壅肺、痰热郁肺、外寒内饮三个证型间临床疗效无显著差异(P>0.05)。结论:应用苈黄导痰汤中西医结合治疗COPD急性加重期疗效良好,其作用机理可能与协同抗感染、缓解气道炎症、改善肺通气与弥散功能等有关。

Objective ：To assess the therapeutic efficacy of Li Huang Dao Tan Decoction on the the acute exacerbation of chronic obstructive pulmonary disease（COPD） and to explore the mechanisms of the effects. Methods ： 125 patients met a predetermined criteria for an acute exacerbation of COPD were consecutively enrolled and randomly distributed to two groups according to the pathogenetie condition. The 63 participants in the treatment group were treated with beth Li Huang Dao Tan Decoction and routine western medicine, the 62 participants in the control group were treated with only routine western medicine. The therapeutic effect, the length of stay, the symptomatie relief time and the improvement of artery blood gas were statistically analyzed and eompared. Results ： Members of the treatment group got better therapeutic effect （ P 〈 0.05, shorter length of stay （ P 〈 0. 05） , shorter symptomatic relief time（ P 〈 0.05,P 〈 0.01）, hetter improvement of P aO2 and P（A-a） O2 （ P 〈 0.05）. Conclusion ：The therapeusis of integrated traditional Chinese and western medicine with Li Huang Dao Tan Decoction has favorable therapeutic efficacy on COPD exacerbation. The contributions of the efficacy might have been anti - infection, releasing tracheal inflammation, improving pulmonary ventilation and alveolar diffusion, synergistically.
