LASIK术后角膜瓣的损伤及处理

Treatment for corneal flap trauma after LASIK.

目的探讨准分子激光原位角膜磨镶术(LASIK)后角膜瓣的损伤及处理方法。方法回顾性分析1997年6月至2005年5月在我院就诊的LASIK术后角膜瓣损伤患者34例(34眼)的临床特点,探讨角膜瓣损伤的处理方法。结果34例角膜瓣损伤均发生于LASIK术后3月以上,平均(14.6±7.2)月。职业:工人16例(47.06%),学生12例(35.29%),军人3例(8.82%)。损伤类型:上皮缺损14例(41.18%),角膜瓣皱褶11例(32.35%),角膜瓣翻卷7例(20.59%),角膜瓣丢失2例(5.88%)。损伤前裸眼视力平均为1.02±0.43,损伤后视力下降至0.48±0.26,经妥善处理后裸眼视力恢复至0.97±0.54。结论LASIK术后数年仍可发生角膜瓣损伤,多见于青年男性。角膜瓣损伤经妥善处理后不致引起严重后果,视力恢复良好。

Objective To explore the clinical characteristic and treatment for corneal flaps trauma after LASIK. Methods 34 eyes of 34 patients with corneal flaps trauma after LASIK surgery enrolled in this retrospective study. The clinical characteristic and treatment were analyzed. Results Corneal flaps trauma of 34 eyes happened 3 months or later after surgery （men 14.6 ± 7.2 months）. Among these 34 patients, 16 cases （47.06%） were workers, 12 cases （35.29%） were students, and 3 cases （8.82%） were soldiers. 14 cases （41.18%） with corneal epithelial defect,11 cases （32.35%） with fold of flaps,7 eases （20. 59% ） with the edge of flaps rolled and 2 cases （5.88%） with loss of flaps were found. The mean uncorrected visual acuity （UCVA） dropped from 1.02 ± 0.43 to 0.48± 0.26 after trauma .The mean UCVA was 0. 97 ± 0.54 after treatment. Conclusions Trauma of corneal flaps can happen even several years after LASIK and were found mostly in young males. After suitable treatment, trauma of corneal flaps will not lead to severe outcomes and the recovery of visual acuity will be. perfect.
