71例儿童急性髓系白血病多参数流式细胞术免疫表型分析 被引量:3

Immunophenotypic analysis of acute myeloid leukemia in 71 children by multi-color flow cytometry

摘要 目的为了解儿童急性髓系白血病(acutemyeloidleukemia,AML)免疫表型特征,分析儿童AML不同亚型抗原表达的差异,并探讨其诊断价值。方法采用三色FCMCD45/SSC双参数散点图设门方法,对71例患者幼稚细胞表面及胞浆内分化抗原进行分析,结合FAB分类进行比较分析。结果在儿童急性髓系白血病患者中,CD33和CD13的表达最常见,分别达97.2%和94.4%,其次为HLADR(69%)。57.7%的病例伴有淋系抗原表达,以CD19表达最常见(36.6%),其次为CD7(28.2%),未见CD20的表达。3例M0患儿经免疫分型确诊。结论结合FAB分类,多参数流式细胞术有助于对AML的准确诊断和分型。FAB不同型别具有不同的免疫表型特征。 Objective To study the characteristics and distribution of childhood acute myeloid leukemia (AML) antigen expressions. Methods The cell-surface and cytoplasmic antigen expressions in 7 1 AML children were analyzed with muhiparameter flow cytometry and CIMS/SSC gating. All patients admitted to our hosopital from Nov 2001 to Jun 2005,50 male and 21 female, aged 2 months to 14 years ( average 6.5 years). The results were compared with the French-American-British (FAB) Cooperative Group classification: M0 3 cases, M1 10 cases, M2 22 cases, M3 4 cases, M4 2 cases, M5 25 cases, M6 2 cases, M7 none. Results Among the 71 cases of AML, CD33, CD13 and HLA-DR were the most commonly expressed antigen (97.2%, 94.4% and 69% respectively). The lymphoid antigen was positive in 57.7% of cases, with the highest frequency of CD19 (36.6%) followed by CD7 (25.2%). There was no CD20 expression among these patients. Three M0 patients were confirmed by immunophenotypic analysis. Conclusion The immunophenotype analysis of AML combined with FAB classification may help in AML diagnosis and typing. Some immunophenotypic features are correlated with FAB type.
