摘要
目的观察CHK1和CHK2在食管癌和癌旁不典型增生组织中的表达及其与食管癌单纯放疗疗效的关系。方法采用WesternBlotting方法检测33例胃镜活检标本诊断为食管鳞癌以及12例癌旁不典型增生组织中CHK1和CHK2蛋白表达,分析单纯放疗的疗效及预后相关因素。结果33例食管癌组织和12例癌旁不典型增生组织中CHK1和CHK2蛋白表达量均无明显差别(P>0.05)。33例食管癌病人单纯放疗后1年生存率为61.44%,中位生存期11.5个月;单因素分析显示年龄、食管造影X线病变长度、CT扫描显示病变长度、临床T分期、N分期、近期疗效以及临床分期均与食管癌放疗预后有关(P<0.05);COX多因素分析仅临床分期为独立预后因素;而食管癌组织中CHK1和CHK2表达对预后无明显影响(P>0.05)。结论CHK1和CHK2不能作为判断食管癌放疗预后的指标,而临床分期则是影响食管癌单纯放疗疗效的主要因素。
Objective To observed expression of CHK1 and CHK2 in tissues of esophageal carcinoma and its precucor and analyzed its relationship with prognosis of esophageal carcinoma after radiotherapy. Methods Protein expression of CHK1 and CHK2 in tissue of 33 cases of esophageal squamous cell earcinoma(ESCC) and 12 cases of dysplasia were detected with western blotting and prognostic related factors in ESCC patients were analyzed after radiotherapy. Results Expression of CHK1 and CHK2 protein were not significant difference between tissues of ESCC and its relevant precursor with semi-quantity analysis respectively. After radiotherapy, 1-year survival rate and median were 61.44% and 11.5 months respectively in 33 cases of ESCC. Age, tumor length in X-ray and CT, T stage, N stage, clinical stage and clinical response were correlated with prognosis of ESCC after radiotherapy with One-way survival analysis; but only clinical stage is the independent prognostic factor with Cox multivariate analysis. Expression of CHK1 and CHK2 protein is not related with prognosis of ESCC after radiotherapy. Conclusion Protein expression of CHK1 and CHK2 was probably not act as prognosis index of ESS after ra- diotherapy, clinical stage is the most important prognostic factor of ESCC after radiotherapy.
