强直性脊柱炎与HLA-B27关联性的分析

The relationship between ankylosing spondylitis and HLA-B27 in Henan province

目的探讨河南地区HLA-B27对强直性脊柱炎的诊断意义。方法对临床疑为强直性脊柱炎患者的静脉血,采用肝素抗凝分离淋巴细胞并用标准的Terasak i软滴法作微量淋巴细胞毒试验。结果疑为强直性脊柱炎患者950例,其中男性660例,女性290例,男女比例为2.28∶1。B27阳性率为51.89%,其中男性占61.82%、女性占29.31%。发病年龄主要集中在青少年时期。结论HLA-B27血清学检测对强直性脊柱炎的诊断和鉴别诊断具有重要意义。

Objective To evaluate the diagnostic significance of HLA - B27 antigen which was used to detect in skeptical ankylosing spondylitis（AS） in Henan province. Methods Peripheral blood mononuclear cells （PBMC） were isolated from venous blood of the patients with skeptical AS by density centrifugation, and the blood samples were anticoagulated with heparinization. The HLA - B27 typing was performed by micro - lymphocyte cytotoxic test with standard soft drop assay. Results Among 950 patients with skeptical AS, male and female were 660 and 290 cases,respectively. The male to female ratio was about 2.28：1 ,and the positive rate of HLA -B27 was 61.82% for male and 29.31% for female. Clinical manifestation of ankylosing spondylitis usually begin in adolescence. Conclusion Detecting HLA -B27 antigen is an effective method on AS diagnosis and differential diagnosis.
