慢性支气管炎急性期痰菌分离及耐药性的检测

The Drug Resistance Tests of the Pathogens Isolated from Patients with Acute Phrase of Nantang Chronic Bronchitis

目的:了解慢性支气管炎急性发作的病原菌及耐药性。方法:对慢性支气管炎急性发作82例患者的痰菌培养和药敏结果进行分析。结果:革兰阴性杆菌51.6%,革兰阳性球菌21.0%,真菌27.4%。在革兰阴性菌中铜绿假单胞菌、肺炎克雷伯菌、大肠埃希菌为主要菌种,对亚胺培南均敏感,革兰阳性菌中以金黄色葡萄球菌、肺炎链球菌为主,未发现耐万古霉素的金黄色葡萄球菌,真菌感染率不断上升。结论:慢性支气管急性发作者多为真菌与耐药菌混合感染,死亡率高。

Objective： To investigate the pathogens isolated from patients with acute phrase of chronic bronchitis and bacterial resistance. Methods. The pathogens isolated from in - patients with acute phrase of chronic bronchitis of our hospital from 2002 to 2004 were cultured. The analysis and the drug - sensitivity tests were performed in every patients expectoration. Results：Among 62 strains 32 （51.6%） were Gram - negative bacilli, 17（21.0% ） were Gram - positive cocii and 17 （27.4%） were fungi. The main strains of Gram- negative bacilli were Pseudomonas aeruginosa, Klebsieua Pneumoniae and Eseherichia coli, and all were sensitive to imipenem. The main strains of Gram- positive cocii were staphylocoeccus aureus and strephococeus pneumoniae, while vancomycin- resistant Staphylocoeccus aureus was not found, The fungi infection increased continuously. Conclusions：Because of complex and severe clinical manifestations, usually accompanied by mixed infection by fungi and drug- resistant bacteria, the prognosis of the patients with acute phrase of chronic bronchitis is po3r. In order to reduce the drug resistance, it is necessary to choase antibiotics correctly according to the results of drug- sensitivity tests instead of the empirical administration.
