胰腺囊性肿瘤的诊断与治疗

DIAGNOSIS AND SURGICAL MANAGEMENT OF CYSTIC NEOPLASMS OF THE PANCREAS

目的：提高对胰腺囊性肿瘤诊断与治疗的认识。方法：对北京医科大学第一医院外科１９６６～１９９４年收治的２０例胰腺囊性肿瘤予以回顾性研究。结果：２０％的患者误诊为假性囊肿并内引流治疗；行根治切除胰腺恶性肿瘤者经１０个月至７ａ不等时间随访，无肿瘤复发。结论：胰腺囊性肿瘤性状常不典型，内覆上皮常剥脱或不连续，即使冰冻活检亦难以在各型肿瘤间进行鉴别。除有典型浆液性囊腺瘤表现者外，均应积极予肿瘤切除，以缓解症状及防治肿瘤恶变，对有孤立转移的囊腺癌亦应积极切除。

Objective . To be misdiagnosed or maltreated. In order to learn more about diagnosis and surgical managements of the desease. Methods. 20 patients with cystic neoplasms of the pancreas who had been treated during who 1 966～1994 period were retrospectively reviewed in this series. Results 20%of the patients had been misdiagnosed as a pseudocyst and were led to improper treatment. The cases with malignant lesions resected for cure are allve without evident recurrence during the follow- up from 1 0 months to 7 years. respectively. Conclusion . The epithelial lining and the components of the tumor were often irregular. and misdiagnoses on gross. parameters were made. Cystic tumors of the pancreas should. in general. be resected for the relief of symptoms and for treatment of cancer. The op-tion of nonintervention may be safe only in those tumors that have the characteristics of serious cvstic adenonma. The cystadenocarcinoma with local metastases should also be resected radicaly.
