内源性抑郁症患者存在脂质代谢异常

Existence of lipid metabolism dysfunction in patients with endogenous depression

目的 探讨抑郁症患者不同亚型之间血脂水平的差异。方法 采用酶法及免疫比浊法检测66例抑郁症和36名健康对照者的血脂[总胆固醇（TC）、三酰甘油（TG）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、载脂蛋白（Apo）A1和ApoB]水平，并比较34例内源性抑郁症患者、32例非内源性抑郁症患者、25例有自杀行为者、41例无自杀行为者与健康对照者血脂水平的差异。结果 内源性抑郁症组血清TC[（3.99±0．76）mmol／L]、HDL-C[（0．96±0．24）mmol／L]、ApoA1[（1．33±0．1]）g/]、ApoB[（0．68±0．11）g／L]均低于对照组，两者比较差异有显著性（P〈0．01～0．001）；非内源性抑郁症组除HDL—C[（1．06±0．23）mmol／L]低于对照组外（P〈0．01），其他差异均无显著性；自杀行为组血清TC[（3．76±0．75）mmol／L]、HDL-C[（1．01±0．26）mmol／L]、LDL-C[（2．32±0．84）mmol／L]、ApoAl[（1．34±0．12）g/L]、ApoB[（0．64±0．14）g／L]均低于正常对照组，两者比较差异有显著性（P〈0．05～0．001）；非自杀行为组除HDL-C[（1．01±0．23）mmol／L]低于对照组外（P〈0．01），其他差异均无显著性。结论 内源性抑郁症患者存在脂质代谢异常，而有自杀倾向的抑郁症患者存在更为严重的脂质代谢异常，其中低HDL-C水平可能是抑郁症共同的生物学标记。

Objective To investigate the difference of the blood lipid levels among patients with different subtype of depression. Methods The serum levels of total cholesterol （TC）, triglycerides （TG）, high-density lipoprotein cholesterol （HDL-C）, low-density lipoprotein cholesterol （LDL-C）, apolipoprotein （Apo） A1 and ApoB in 66 patients with depressive disorder and 36 healthy controls were measured by enzymatic methods and immunoturbidimetry. The blood lipid levels were compared among the 34 patients with endogenous depression, 32 patients without endogenous depression, 25 patients with suicide behavior, 41 patients without suicide behavior and the healthy controls. Results The serum levels of TC[（3.99 ±0.76） mmol/L], HDL-C[（0.96 ±0.24） mmol/L], ApoA1 [（1.33 ±0.11） g/L] and ApoB [ （0.68 ±0.11 ） g/L] were significantly lower in patients with endogenous depression than those in healthy controis （P 〈 0.01 ～0. 001 ）. In patients with non-endogenous depression, only serum HDL-C [ （ 1.06 ±0.23 ） mmoL/L ] level was significantly lower than that in healthy controls （P 〈 0.01 ）, and there was no other significant difference. The serum levels of TC [（3.76±0.75） mmol/L], HDL-C [（1.01 ±0.26） mmol/L], LDL-C [（2.32 ±0.84） mmol/ L], ApoA1 [ （1.34 ±0.12） g/L], and ApoB [ （0.64±0.14） g/L] in patients with suicide behavior, were significantly lower than those in healthy controls （P 〈 0.05 ～ 0. 001 ）. in patients without suicide behavior serum HDL-C level [ （ 1.01 ±0.23） mmoL/L] was significantly lower than that in healthy controls （P 〈0.01 ）, and there was no other significant difference. Conclusions The patients with endogenous depression have lipid metabolism dysfunction. The patients with suicide behavior have severe lipid metabolism dysfunction. The low HDL-C level may be the common biological marker in depression.
