摘要 目的:通过对肘内侧副韧带的解剖,为临床诊治肘关节内侧不稳定提供科学依据。方法:解剖40例成人尸体肘关节标本,观察肘内侧副韧带组成及形态结构特点,并测量肘关节在不同屈曲角度下韧带成分的长度。加外翻力矩,测量有完整韧带、切断前束、切断后束、切断前后束肘外翻角度的变化。结果:(1)肘内侧副韧带由前、后、斜束组成。(2)前、后束的长度随肘屈曲角度的变化而变化,斜束的长度不随肘关节屈曲角度的变化而变化。(3)加外翻力矩切断前束、切断前后束以及切断后束在肘关节屈曲大于90度时,肘外翻角度变化有统计学意义。结论:肘内侧副韧带由前、后、斜束组成。肘内侧副韧带前束是维持肘外翻稳定的重要结构。 Objective:The purpose of this study is to provide the anatomical basis for clinical diagnosis and treatment of the medial instability of the elbow. Methods :The ligament composition, shape and motion of the elbow in 40 cadaveric specimens were studied in dissection and the length of each ligament component was measured at each of angle of flexion. The valgus degree of the elbow was measured at different angle of flexion, and the valgus torque was given to the elbow on the testing apparatus, the medial collateral ligament compositions were cut serially to study the stability of the elbow at each of angle of flexion. Results : ( 1 ) The medial collateral ligament of the elbow was composed of an anterior bundle, a posterior bundle and a transverse bundle. (2) The length of the anterior bundle and the posterior bundle varied with the change of the angle of elbow flexion. 3. The mean length of the transverse bundle was of no change of the angle of elbow flexion. When the valgus torque was given to the elbow, cutting of the anterior bundle and cutting of both the anterior nd the posterior bundle would he found significantly to increase the degree of the valgus angle of the elbow, respectively, compared with the intact elbow. But when only the posterior bundle was cut before 90 degree of flexion, there were no significant increase in valgus angle of the elbow. Conclusion : 1. The medial collateral ligament of the elbow is composed of an anterior bundle, a posterior bundle and a transverse bundle,in which the anterior bundle is the most important element. 2. The anterior bundle is the primary restraint to valgus stress and should be the focus of a reconstructive operation .
