高频超声和彩色多普勒超声在诊断腹型过敏性紫癜中的价值

Evaluation of High-frequency Ultrasound and Color Doppler Flow Imaging for Diagnosis of Abdominal Type Allergic Purpura

目的探讨高频超声和彩色多普勒超声在腹型过敏性紫癜中的诊断价值。方法运用高频超声和彩色多普勒超声对25例急腹症患儿进行腹部肠道超声检查,观察受损肠壁回声、厚度,测量受损肠壁的彩色血流的速度及阻力指数。结果25例超声诊断为腹型过敏性紫癜的患儿经临床、实验室、内镜检查确诊为腹型过敏性紫癜的有24例。超声诊断符合率为96%,其受损肠壁声像特点:1.肠壁单发或多发性、节段性增厚;2.增厚肠壁血流丰富;3.增厚肠壁的血流速度及阻力指数(RI)均较正常稍有下降。结论高频超声及彩色多普勒超声在儿童腹型过敏性紫癜的早期诊断及鉴别诊断起重要作用。

Objective To evaluate high-frequency ultrasound （HFUS） and color Doppler flow imaging （CDFI） in the diagnosis of abdominal type allergic purpura. Methods A total of 25 children with acute abdominal pain were examined with HFUS and CDFI. The echoes and depth of the injured intestine were observed; the blood flow velocity and RI were measured. Results Twenty-four cases were confirmed by clinical, laboratory and endoscopic examinations. The diagnositic accordant rate was 96% （24/25） . The characteristics of the injured wall were ：single or multiple areas of intestinal wall thickening with plenty of blood flow. The blood flow velocity and RI in the injured intestine were decreased slightly. Conclusions HFUS and CDFI have important value for early diagnosis and differential diagnosis of abdominal type allergic purpura.
