彩色多普勒超声对妊娠期子宫动脉血流阻力评分与妊娠结局相关性的探讨 被引量:4

摘要 目的探讨彩色多普勒超声对妊娠妇女子宫动脉血流阻力评分及与妊娠结局的比较。方法选择妊娠23-42周妊娠妇女109例,分别测量双侧子宫动脉S/D值(收缩期和舒张期比值)并根据S/D值的高低及有无舒张期切迹进行评分。按评分高低将研究对象分为3~4分组、1~2分组和0分组3个组。评分结果与妊娠结局加以比较。结果3~4分组其孕周、胎儿出生体重、百分比体重均明显小于1~2分组和0分组。而3~4分组的新生儿出生后1min评分小于7分、出生时面罩吸氧、及合并妊娠高血压综合征、先兆子痫及IUGR(胎儿宫内生长迟缓)的比率均显著高于其他两组。结论经彩色多普勒超声对双侧子宫动脉阻力指数进行测量并评分可以预测妊娠结局。 Objective To study the uterine artery score of pregnant women and to explore the association to the adverse perinatal outcomes. Methods In 109 pregnant women,the uterine artery score was measured according to the ratio of systolic to diastolic velocity (S/D) in bilateral uterine arteries and the presence of notches in the spectra was observed. We divided the pregnancy women into three groups: 3-4 points (A). 1-2 points (B) and 0 point (C)groups. The scores were compared with the perinatal outcomes. Results The gestational weeks, the birth weight and the percentage of birth weight in group A were significantly lower than those in group B and C. The neonatal 1-minute Apgar score, resuscitation with mask. pregnancy-induced hypertension, preeclampsia and IUGR (intra-uterine growth restriction) in group A were significantly higher than those the of the other two groups. Conclusions In second and third-trimester pregnancy, color Doppler velocity waveforms of bilateral uterine arteries can be evaluated with the uterine artery score, which possesses high predictive value regarding adverse perinatal outcomes.
