高强度聚焦超声在良性肿瘤中的应用分析

High Intensity Focused Ultrasound in Treatment of Benign Neoplasms

目的探讨高强度聚焦超声(HIFU)治疗良性占位性疾病的价值、疗效和不良反应。方法选择拒绝或惧怕手术的子宫肌瘤、前列腺肥大、乳腺纤维腺瘤或结节性增生,卵巢实性占位等患者,采用HIFUNIT9000型超声聚焦肿瘤消融机,在B超定位并监视下,通过6个换能器三维适形优化组合治疗方案,使靶区点点成线,线线成面,面面成体逐渐固化覆盖瘤体。结果476例患者(肌瘤370例410个瘤灶、子宫腺肌症50例、前列腺肥大26例、卵巢实性占位10例,乳腺纤维腺瘤或结节增生20例),治疗后瘤体内回声较前增高不均,瘤体内血流信号减少或消失,体积逐渐缩小,临床症状改善或恢复正常。随访1年以上,体积明显缩小或消失37%,缩小46.8%,无变化16%,增大0.2%。结论HIFU治疗良性占位或前列腺增生病,是一种无创无辐射有效安全的方法,值得推广应用。

Objective To explore the value,curative effect and adverse reaction of high intensity focused ultrasound(HIFU)in treating benign neoplasms.Methods Patients with hysteromyoma,benign prostatic hyperlasia,breast fibroadenoma or tubercular hyperplasia,ovary occapying diseases were selected.Using NIT9000 and optimalized treating project with 6 transducers and 3-D,type.HIFU covered the tumor under the monitoring of B ultrasound.In the focuses,dot connected dot into line,lines into surface and surfaces into the stero whole neoplasms were ablated.Results The echoes were increased more unequally than before in the tumors Of 476 patients after treatmemt (370 myoma patients including 410 tumor focuses,50 cases of adenomyosis,26 of benign prostatic hyperplasia,10 of ovary occupying disease and 20 of breast fibroadenomaor tubercular hyperplasia).The blood signals in tumors were decreased or vanished,the volume was reduced gradually and the clinical symptoms were improved or recovered entirely.Following up more than one year,the volume was decreased clearly or vanished in 37%,decreased or became shrinking in 46.8%,no change in 16% and became larger in 0.2%.Conclusions HIFU is a noninvasive,non-radiated,effective and safe technigue in treating benign neoplasms or benign prostatic hyperplasia.It is worthy of further clinical application.
