不同阴性症状对精神分裂症诊断价值的比较研究

Value of different negative symptoms in the diagnosis of schizophrenia.

比较了情感平淡、思维贫乏、意志缺乏、愉快感缺乏及注意障碍５项阴性症状对精神分裂症的诊断价值发现，思维贫乏的特异性最高，情感平淡的敏感性、总效率和与ＣＣＭＤ—２诊断精神分裂症的一致性最高，但均不很理想。

Objective:To study the value of different negative symptoms in the diagnosis of schizophrenia.Method:The diagnostic value of the negative symptoms of apathy poverty of thought,abulia,anhedonia and in attention were compared.Results:The value of different negative symptoms in the diagnosis of schizophrenia was different.The symptom with highest specificity was poverty of thought;that with highest sensitivity and kappa was apathy.The value of negative symptoms in the diagnosis of schizophrenia was not too satisfactory.Discussion:The manifestation of negative symptoms is complex.Apathy may occur after the disappearance of positive symptoms.Anhedonia is difficult to distinguish from depressed mood,and overlaps with post-schizophrenic depression,post-psychotic deterioration,tardive dyskinesia,depression due to drugs and institutionalized states.The diagnostic value of negative symptoms in schizophrenia is therefore limited.
