电针内关穴后下丘脑室旁核、β-内啡肽及白细胞介素1的变化 被引量:9

摘要 目的:观察电针刺内关穴后下丘脑室旁核及该区β-内啡肽、白细胞介素1的变化。方法:实验于2004-06于湖北中医学院针灸实验室完成。选取8~12月龄大耳白家兔56只,雌雄各半。随机分为急性心肌缺血+电针组、下丘脑室旁核损毁+急性心肌缺血+电针组、假损毁+急性心肌缺血+电针组、下丘脑室旁核损毁+急性心肌缺血组、下丘脑室旁核损毁模型组、急性心肌缺血模型组、正常组,共7组,每组8只。采用左冠状动脉前降支结扎法制备家兔急性心肌缺血模型,观察电针内关穴对心电图ST段电位的影响;同时观察下丘脑室旁核电解损毁预处理对心电图ST段电位的影响;并且通过免疫组化方法观察室旁核内β-内啡肽、白细胞介素1表达变化。结果:①急性心肌缺血+电针组、下丘脑室旁核损毁+急性心肌缺血+电针组、假损毁+急性心肌缺血+电针组ST段回复的时程及幅度显著快于下丘脑室旁核损毁+急性心肌缺血组(P<0.05)。急性心肌缺血+电针组、假损毁+急性心肌缺血+电针组又优于下丘脑室旁核损毁+急性心肌缺血+电针组(P<0.05),下丘脑室旁核电解损毁导致家兔出现一过性轻度心肌缺血,下丘脑室旁核预先电解损毁显著降低电针内关穴抗急性心肌缺血损伤效应。②急性心肌缺血+电针组、假损毁+急性心肌缺血+电针组、急性心肌缺血模型组β-内啡肽、白细胞介素1蛋白表达阳性率与正常组比较,差异显著(P<0.05~0.01)。结论:①电针内关穴能显著降低实验性急性心肌缺血家兔心电图ST电位,抗急性心肌缺血损伤。②下丘脑室旁核参与内关-心脏相关的中枢通路,损毁室旁核会在一定程度上显著降低电针内关穴抗急性心肌缺血的效应,室旁核内β-内啡肽、白细胞介素1可能参是电针内关作用的中枢效应物质。 AIM: To study the effect of acupuncturing at Neiguan point on changes of hypothalamic paraventricular nucleus (HPN), β-endorphin and interleukin-1 (IL-1). METHODS: The experiment was completed in the Laboratory of Acupuncture and Moxibustion of Hubei College of Traditional Chinese Medicine in June 2004. Totally 56 rabbits aged 8-12 months and of half gender were divided randomly into 7 groups: acute myocardial ischemia (AMI) + acupuncture group, HPN damage + AMI + acupuncture group, sham damage + AMI + acupuncture group, HPN damage + AMI group, HPN damage model group, AMI model group and normal group with 8 in each group. AMI models of rabbits were established with ligation of anterior descending branch of left coronary artery so as to observe the effect of acupuncturing at Neiguan point on ST potential of electrocardiograph (ECG), the effect of pretreatment of HPN-electrolyte damage on ST potential of ECG, and changes of β-endorphin and IL-1 in HPN with immunohistochemical method, RESULTS: ① Reversed time and extent of ST segment were faster in AMI + acupuncture group, HPN damage + AMI + acupuncture group and sham damage + AMI + acupuncture group than those in HPN damage + AMI group (P 〈 0.05), but those in AMI + acupuncture group, sham damage + AMI + acupuncture group were superior to those in HPN damage + AMI + acupuncture group (P 〈 0.05). HPN-electrolyte damage could induce transient mild AMI and HPN-electrolyte damage could relieve the effect of acupuncturing at Neiguan on AMI injury. ② Positive rates of β-endorphin and IL-1 protein in AMI + acupuncture group, sham damage + AMI + acupuncture group and AMI model group were significantly different from those in normal group (P 〈 0.05-0.01). CONCLUSION: ① Acupuncturing at Neiguan point can decrease ST potential of ECG and resist AMI injury of AMI rabbits. ② PVN involves in central passage from Nelguan to heart and damage of paraventricular nucleus can reduce the effect of acupuncturing at Neiguan point on AMI in a certain degree. β-endorphin and IL-1 in paraventricular nucleus may be the central effectors during acupuncturing at Neiguan point.
