摘要
目的探讨胺碘酮在预防心房颤动(房颤)发作时对心电图P波离散度的影响及作用机制。方法将80例冠心病阵发性房颤患者随机分为两组,40例在常规治疗基础上加用胺碘酮治疗(治疗组),40例只接受常规治疗(对照组),随诊3个月,分别于治疗前及治疗后1个月、2个月、3个月行心电图检查,测量其最大P波(Pmax)时间,最小P波(Pmin)时限及P波离散度(Pdis)。结果治疗组3个月后Pmax无明显变化,Pmin增大,Pdis缩小,而对照组Pmax、Pmin及Pdis无明显变化。两组间Pmin差异有显著意义(P<0.005),Pdis差异有显著意义(P<0.001)。结论胺碘酮使Pdis缩小可能是其有效预防房颤复发的机制。
Objective To study the effects and mechanism of amiodarone on P wave dispersion in preventing paroxgsmal atrial fibrillation. Methods 80 cases with coronary heart disease and paroxysmal atrial fibrillation were randomly dividedl into two groups,study groups were given amiodarone and general treatments of coronary heart dis- ease,control group only with general treatments, to measure the maximal P wave duration (Pmax), the minimal P wave duration(Pmin) and P wave dispersion(Pdis), before administration and when administration one month, two months and three months, respectivery. Results Pmin. increased, Pdis got small after 3 months of adiministration of amiodarone. Compared with control group, the Pmin Of electrocardiographry of study group was obvionsly different( P 〈 0. 005), so was Pdis,( P 〈 0. 001 ). Conclusion Amiodarone may effectively prevent the onset of atrial fibrillation by reducing Pdis.
