血管内支架植入联合弹簧圈栓塞治疗颅内复杂动脉瘤(附15例报告)

Effects of combining endovascular stents and coils for complex aneurysms

目的探讨血管内支架植入联合弹簧圈栓塞治疗颅内复杂动脉瘤的疗效、技术要点、安全性及并发症防治。方法研究对象为9例宽颈动脉瘤、5例夹层动脉瘤及1例梭形动脉瘤。其中6例采用Neuroform支架。9例使用BX支架，首先将支架跨动脉瘤颈放置．通过支架上的网眼将微导管送入动脉瘤腔内。填塞弹簧圈。结果15例手术均成功，术后动脉瘤达到致密填塞，载瘤动脉畅通。造影随访7例（随访期6～18个月）。未见有动脉瘤显影。载瘤动脉畅通。结论血管内支架植入联合弹簧圈栓塞治疗颅内复杂动脉瘤安全有效。

Objective： To explore the efficacy, technical tip, safety and complication of combining endovascular stents and coils for complex aneurysms. Methods： Among 9 cases wide neck aneurysms, 5 cases dissecting aneurysms and 1 case fusiform aneurysm, Neuroform stents were used in 6 cases,and BX stents in 9 cases. Implanted the stents which spanned the neck of the aneurysms, and then released the coils by microcatheters which were put into the aneurysms through stent mesh. Result： The operation was succeeded in 15 cases. It meaned the aneurysms were packed totally, and the parent artery was kept patent. By the follow-up angiography in 7 cases （6-18 months）, there was no opacification of aneurysm was found and the parent artery was patent. Conclusion： Combined endovascular stent and coil is a safe and effective method for the treatment of intracranial complex aneurysms.
