腹腔镜技术在结直肠癌的临床应用（附11例临床报告）

Usage of laparoscope in colorectal cancer： report of 11 cases

目的：总结腹腔镜外科技术在大肠肿瘤的临床应用效果。方法：收集该科2005年1月至2005年8月收治的11例大肠肿瘤行腹腔镜手术，男5例，女6例，其中年龄36-78岁，术前诊断直肠癌9例，结肠脾曲癌1例、结肠肝曲癌1例；9例直肠癌5例根治（4例行Dixons、1例MiLes），4例直肠癌为姑息手术行乙状结肠双腔造瘘；1例腹腔镜辅助下右半结肠切除；1例晚期结肠脾曲癌肝转移行横结肠一乙状结肠侧侧吻合术。结果：全组均成功完成手术，其中1例中转手术，术中无大出血、无输血，无手术并发症，手术时间40～360min，平均165min，术后下床活动时间24～48h，肠功能恢复时间24～60h。结论：只要掌握腹腔镜高级操作技术，具备腹腔镜重要手术器械（超卢止血刀，双吻合器操作），注重无瘤技术，大肠肿瘤腹腔镜手术是可行，有效，值得推广应用。

Objective： To evaluate the clinical effect of laparoscope using in colorectal cancer. Method： From Jan 2005 to Aug 2005 ,operations under the laparoscope were used in 11 cases. In this study, five patients with rectal cancer underwent radical cure and four did palliative operation .One patient with hepatic flexure of Colonic cancer underwent right hemicolonic excision and the other with splenic flexure of colonic cancer did side-to-side anastomosis of transverse colon and sigmoid colon. Result： All operations were performed success＂ fully and one case was changed into routine operation . No complication was observed and no cases were transfused blood. The time of operations was from 40 to 360 min. Postoperational movements of patients were done from 24 to 48 h, and flatus and feces were passed from 24 to 60 h. Conclusion： Laparoscope using in colorectal cancer is effective and is an ideal choice.
