靶向持续性组织间化疗治疗舌鳞癌的初步观察

Primary study of the tongue squamous cell carcinoma treated with targeting and continuous interstitial chemotherapy

目的观察靶向持续性组织间化疗治疗舌癌的疗效及安全性。方法将顺铂(缓释型)植入于10例舌癌患者癌区周围及中央,观察局部变化、全身反应与术后标本的病理结果。结果所有患者原发灶都有不同程度的缩小,部分缓解8例,无效2例(缩小约30%左右),有效率为80%。所有患者术后标本肉眼见肿块边缘药物所在区形成比较明显的纤维组织样反应;镜下见癌中央组织凝固性坏死,植入药物周围组织变性、坏死,转移淋巴结内可见部分坏死灶、组织变性。结论顺铂(缓释型)治疗舌鳞癌,疗效较好,操作简单,安全性高,毒副作用少,对转移淋巴结有一定的治疗作用。

Objective：To observe the efficacy and side effect of targeting and continuous interstitial chemotherapy in the treatment of tongue squamous cell carcinoma. Methods：Sustain-released cisplatin（20 mg/piece） was implanted into and around tongue squamous cell carcinoma in 10 cases,20 mg was used for the tumor at T1,40 mg for T2 and 60 mg for T3.7 - 10 days after implantation tumor resection was performed, the efficacy and side effect were observed. Results ：The size of all tumor were reduced,50% -60% reduction was observed in 8 cases. Pathological examination of after-operation samples showed cancer cell necrosis in the center, and the degeneration and necrosis in the peripheral tissues where the drug had been implanted in. Necrosis and tissue degeneration could be seen in the metastasized lymph nodes. The side effects were local pain, hyperemia and edema in all the cases. Slight high temperature and nausea were observed in a few cases. Conclusion：The sustain-released cisplatin implanted into lesions is effective for the treatment of squamous cell carcinoma of the tongue with no severe side effects.
