摘要
目的:利用经动脉性门静脉造影CT重建门静脉、肝静脉三维结构,观察生理状态下的门静脉的解剖与变异。方法:150例病人,导管置入于肠系膜上动脉内,注入造影剂后门静脉期和肝静脉期连续扫描肝脏。三维重建门静脉及肝静脉,分析门静脉的解剖与变异。结果:150次成像中门静脉变异25例,12例(8.0%)显示门静脉呈三分叉状,10例(6.7%)门静脉先分出右后支,然后上行分为左支和右前支,1例(0.7%)门静脉左支水平段缺如,门静脉右支缺如2例(1.3%),余下125例(83.3%)显示正常左右门静脉分支。结论:门静脉的三维图像重建及类型分析对术前手术方式的确定有一定的临床意义。
Objective: To reconstruct 3-dimentional portal vein and hepatic veins by computed tomography arterial portography(CTAP) and determine the variations of the portal vein and hepatic veins at physiological state. Mothods: Livers of 150 patients were continuously scaned at the intrahepatic portal vein and hepatic vein phases after injecting contrast medium into superior mesenteric artery through catheter. The variations of the portal vein were observed after reconstructing 3-dimentional portal vein and hepatic veins. Results: There were variations of 25 portal veins in 150 patients examined. 12 cases (8.0%) showed trifurcation, 10 cases (6.7%) showed a right posterior branch arising from the main portal vein first and then the right anterior and left portal vein originated from the ascending main portal vein, 1 case (0.7%) absent the horizontal segment of the left portal vein and 2 cases (1.3%) absent right portal vein. The remaining 125 (83.3%) patients appeared normal distribution of the portal vein. Conclusions:Reconstruction and type analysis of 3-dimentional portal vein is significant for determining operational methods in clinic.
