宫腔镜检查与子宫内膜癌细胞扩散相关性探讨

目的探讨宫腔镜检查对子宫内膜癌患者腹腔冲洗液癌细胞的检出率是否有影响。方法回顾性分析经宫腔镜检查和经分段诊刮为子宫内膜癌的腹腔冲洗液检查结果共176例。结果经宫腔镜检查的89例患者中,腹腔冲洗液的阳性率(21·7%)高于经分段诊刮诊断得的(14·4%),但差异无统计学意义。在肌层浸润>1/2的子宫内膜癌患者中,经宫腔镜诊断者其腹腔洗液阳性率(26·6%)明显高于经分段诊刮诊断者(7·3%)。结论宫腔镜检查用于诊断子宫内膜癌时,有可能引起癌细胞的扩散。

Objective To study whether hysteroscopy affects the result for peritoneal cytology of patients with endometrial carcinoma. Methods The peritoneal cytology of 176 patients with endometrial carcinoma diagnosed by D＆C or hysteroscopy were retrospectively analysed. Results The peritoneal cytology was positive in 21.7% of the endometrial cancer patients, diagnosed by hysteroscopy, which was higher than that 14.4% of the patients diagnosed by D＆C while no statistical difference was found when analyze the patients with involvement of inner more half of the myometrium, the positive rate of peritoneal cytology of patients diagnosed by hysteroscopy （26.6 % ） was sigificantly higher than that（7.3% ） of the patients diagnosed by D＆C. Conclusion Hysteroscopy used in the diagnosis of endometrial carcinoma may facilitate the tumor cell dissemination.
