犬辅助性原位肝左叶移植模型的建立 被引量:3

Experimental canine model of auxiliary orthotopic liver left lobe transplantation

摘要 目的建立稳定可靠的辅助性肝移植动物模型。方法杂交犬14条8-25 kg随机分两组。供体组肝脏均采用尸体肝左叶;受体组行标准左叶切除,供肝左叶原位移植于受体体内。结果供肝热缺血时间为零,冷缺血时间平均36.3 min;灌注液的量平均2.96 L,切取修剪肝左叶时间为23-40 min。受体手术平均时间5.3 h,平均出血量140 ml,肝脏血管重建后红润柔软,5-11 min内即见胆汁从胆管中溢出。受体组术后全部存活,围手术期未用任何血管活性药物、抗生素及免疫抑制剂,术后存活超过6 h者5例,最长存活者达5.7 d。结论犬是建立辅助性肝移植模型的理想动物。 Objective To establish the model of auxiliary orthotopie liver left lobe transplantation in dogs. Methods Fourteen dogs were randomized into two groups. Dogs in donor group undertook standard left hepatic lobeetomy, liver pea-fusion in situ and whole liver resection, and the left lobe was reserved for grafting into the recipients. Hepatic vein, portal vein and bile duct were anastomosed, and hepatic artery of donors was anastomosed with gastroduodenal artery of recipients. Results In cadaver donor group, warm ischemia time was zero and average cold ischemia time was 36.3 min (27-45 min), and mean quantity for perfusion was 2.96 L ( 1.75-4.5 L), time of liver left lobe scissoring ranged from 23 min to 40 min. In recipient group, average operation time was 5.3 h, average blood loss was 140 ml (70- 190 ml), bile effusing from bile duct could be seen during 5 rain to 11 min, all dogs in recipients survived after grafting, no vssodilator, antibiotics and immunosuppressor were used perioperatively, 5 dogs had a survival time of over 6 h after operation and the longest survival time was 5.7 days in one dog. Conclusion Experimental canines are the ideal animals developing model of auxiliary orthotopic liver left lobe transplantation.
