IgA肾病患者肾组织中增殖细胞核抗原表达

Expression of PCNA in the renal tissues from the patients with IgA nephropathy

用免疫组化方法研究了２０例原发性ＩｇＡ肾病患者肾组织中增殖细胞核抗原（ＰＣＮＡ）表达。结果显示ＩｇＡ肾病患者肾组织中ＰＣＮＡ表达增加，肾小球内ＰＣＮＡ阳性细胞数、肾小管和间质中ＰＣＮＡ阳性细胞百分数均与肾组织学损害程度呈正相关；临床－病理研究显示肾小管中ＰＣＮＡ阳性细胞百分数分别与２４小时尿蛋白量、血清肌酐浓度（Ｓｃｒ）呈正相关；肾间质中ＰＣＮＡ阳性细胞百分数亦与２４小时尿蛋白量呈正相关。肾脏细胞增殖程度作为一项判断肾小球肾炎组织学损害程度和预后的指标值得进一步研究。

Paraffin-embedded renal tissue sections from 20 patients with IgA nephropathy were examined byusing immunohistochemical staining(LSAB methord)with antiproliferating cell nuclear antigen(PCNA)mono-clonal antibody(Clon PC10).For the six control sections,PCNA+cells were found occasionally in tubules butwere not detected in glomeruli.For all sections from the patients,PCNA+cells were demonstrated in glomeruli,tubules and interstitium. The mean numbers were 1. 34±0.63 per glomerulus,42. 35±12.39 per 100 tubular ep-ithelial cells and 12.35 ± 4.20 per 100 interstitial cells,respectively.The numbers of PCNA+cells in glomeruli,tubules and interstitium correlated with histological grade,rs’=0.4718(P<0. 05),rs’=0.8157(P<0.001)andrs’=0.7573(P<0.001),respectively. Clinicopathological study showed PCNA+cell percentage in tubules corre-lated with both serum creatinine(Scr)(r=0.4653,P<0.05)and 24 hour urinary protein(r=0.6618,P<0.02).A positive correlation was also found between PCNA+cell percentage in interstitium and 24-hour urinary protein(r=0.5007,P<0.05).But the number of PCNA+cells per glomerulus did not correlate with Scr and 24-hour uri-nary protein,In summary,PCNA is overexpression in the renal tissues from the patients with IgA nephropathyand renal cellular proliferation indices could be useful in grading renal damage histologically and predicting progno-sis of the disease.
