普通外科监护病房医院获得性感染细菌培养与药敏分析

摘要 目的:了解普通外科监护病房(ICU)医院获得性感染常见细菌及药敏结果,以指导临床合理用药。方法:采用VITEK全自动微生物鉴定仪对73例普通外科ICU患者采集97株细菌进行鉴定及药敏分析。结果:感染部位以呼吸道感染发生率最高,占40.2%,其中以肺炎克雷白杆菌致病占首位。泌尿系感染、胃肠道和腹腔感染多为大肠埃希氏菌。切口感染最常见细菌为假单胞菌属。药敏结果显示革兰氏阴性菌株对亚胺培南敏感,革兰氏阳性菌株对万古霉素、替考拉宁均敏感,对3代头孢及左氧氟沙星部分敏感,对1代头孢及氨苄青霉素均耐药。结论:对普通外科ICU细菌菌株及耐药性的动态监测,合理应用抗生素,根据细菌培养与药敏结果选择针对性强、抗菌谱窄的抗生素对防治感染是非常重要的。 Objective: To study the pathogenic bacteria and their drug sensitivity in general surgical intensive care unit (SICU) so as to guide reasonable clinical medicine application. Methods: Using VITEK automicrobial analysis instrument, the 97specimens from 73 patients were collected and cultured at same time, the types of bacteria and their drug sensitivity were identified. Results: Among 97 specimens, respiration tract infection was 40.2%,most of the pathogenic bacteria was klebsiella pneumoniac. Most of bacteria in digestive and urinary system were escherichia coli. The results of drug sensitivity indicated pathogenic bacteria were sensitive to imipenem and vancomycin, partly sensitive to third generation cephalosporine and levofloxacin. Conclusion: The tendency to antimicrobial resistance of clinical isolates should be followed up continuously. Selection of antimicrobial should be based on result of bacteria culture.
