摘要
随着外科技术和设施的不断发展，脱泵下冠状动脉搭桥术（OPCABG）呈逐渐增多的趋势。完善的术前准备，缓慢平稳的麻醉诱导，术中血流动力学维持平稳，术者操作轻柔及合理的血管吻合顺序，经食道超声心动图（TEE）及主动脉动囊反搏（IABP）使用等均为OPCABG成功的关键。本文拟从冠心病病人术前病情特点、麻醉方面的要求，术中监测、术中血流动力学变化的维护、心肌缺血、预防、血流动力学支持的应激措施及术后镇痛等方面的进展作一综述。
The patients undergoing off-pump coronary artery bypass grafting （OPCABG） are increasing with the development of surgical techniques and devices. The keys of success for OPCABG are perfect pre-operative preparations and anaesthetic induction, stable hemodynamics during operation, gentle cardiac manipula- tion, myocardial ischemia prevention and the use of intra aorta balloon pump （IABP） and transesophagus echocardiography （TEE）. Anesthesiologists involved in cardiac surgery must have knowledge of this specific procedure and its requirements, and to prevent and treat the hemodynamic instability and severe heart complications. This article will review the progresses of all problems above.
