直肠癌的介入治疗

Interventional therapy of rectal carcinoma

目的：评价直肠癌选择性动脉灌注化疗和栓塞的临床价值。方法：对32例直肠癌患者（29例行手术切除）中，12例术前采用Seldinger技术行选择性灌注化疗并部分用明胶海绵栓塞，3～10d内行直肠癌手术，17例术后行灌注栓塞，3例已有肝转移者只行灌注栓塞。结果：灌注栓塞后患者临床症状明显缓解。术前行介入治疗的患者中，10例行根治性切除（10／12），且术中出血明显减少。结论：直肠癌的动脉灌注化疗及栓塞安全有效，可提高手术切除率及减少术中医源性扩散和术后的转移、复发。

Objective： To evaluate the clinical value of selective arterial infusion chemotherapy and embolization for rectal carcinoma. Methods： Among 32 patients of rectal carcinoma, 29 patients underwent operation, 12 cases received infusion chemotherapy and embolization by Seldinger technique before operation, 17 cases received infusion chemotherapy after operation, and 3 cases with liver metastasis received infusion chemotherapy only. Results： The clinical symptoms were obviously improved in all patients. A total of 10 patients received radical surgery and the hemorrhage was obviously reduced during the operation. Conclusion： The arterial infusion chemotherapy and embolization is safe and effective. It may increase the resection rate and reduce the metastasis and recurrence in rectal carcinoma.
