蝎素抗癌因子胸腔内给药治疗癌性胸水23例近期疗效观察

Treatment of malignant pleural effusion with intrapleural injection of AFSVC-Ⅱ

为探讨蝎素抗癌因子（ＡＦＳＶＣ－Ⅱ）对癌性胸水的治疗作用，将癌性胸水患者随机分为治疗组２３例，对照组２０例，两组性别、年龄、病程、病理分型等构成比大致相同。在治疗前均未对胸水进行任何治疗，每次胸腔给药时，均在抽尽胸水后进行。治疗组用ＡＦＳＶＣ－Ⅱ０．４ｍｇ加２ｍｌ注射用水稀释后注入，每周２次，每周增加０．４ｍｇ，最大剂量≤１．６ｍｇ。对照组用５－氟尿嘧啶５００ｍｇ，丝裂霉素１６～２０ｍｇ，每周２次，４周后进行比较。结果：治疗组对腺癌引起的胸水的疗效优于对照组（Ｐ＜０．０５），对疼痛、纳差（Ｐ＜０．０１），增加外周白细胞均有显著差异（Ｐ＜０．０５）。提示ＡＦＳＶＣ－Ⅱ对肺癌伴胸水有效。

To assess the effects of antitumor factor of scorpion venom component Ⅱ(AFSVC-Ⅱ) in the treatment of malignant pleural effusion, 23 patients were taken in AFSVC-Ⅱ group and 20 in the control group.The patients in the two groups were comparable in sexes, ages, pathological types and the intervals between the diagnosis and the treatment.All the patients were unintervented before entering the study. The pleural injection therapies were proceeded by completely sucking the pleural fluid.In the AFSVC-Ⅱ group,0.4 mg of AFSVC-Ⅱdiluted in 2 ml sterile water were injected twice a week,and the dose was doubled weekly with the maximum dose of 1.6mg. In the control group 5 00 mg of 5-Fu or 16～20 mg of MMC were injected twice a week.The two groups were compared four weeks later. The results showed that in the treatment of pleural effusion-caused by adenocarcinoma, AFSVC-Ⅱ was more effective than the control agents (P<0.01).In relieving of the chest pain,anorexia and the increase of peripheral WBC,the differences were significant between the two groups(P<0.05).It is concluded that AFSVC-Ⅱis effective in the treatment of malignant pleural effusion and it is superior in improving the patient′s life.
