腹主动脉瘤及胸腹主动脉瘤

Abdominal and Thoracoabdominal Aortic Aneurysm

本文总结了１９８６～１９９５年北京协和医院收治的４２例腹主动脉瘤及７例胸腹主动脉瘤的治疗经验。有３７例男性１２例女性，年龄平均６５岁，病程从２周至１０年。８例做ＣＴ检查，４０例做超声检查，４３例动脉造影，经食道超声５例，治疗方式是行动脉瘤切开人工血管再建４０例无手术死亡。胸腹主动脉瘤手术４例，１例死于低温致室颤，３例采用分次阻断主动脉法减少内脏缺血时间，手术成功，作者认为对动脉瘤仍应手术治疗。术前应重视诊治伴随疾病，改进手术技巧，注意围手术期处理提高手术成功率。

From 1986 to 1994,49 patients with Abdominal or thoraco-abdominal aortic aneurysm(AAA &TAAA)were admitted to PUMC hospital.There were 37 males and l2 females.Age ranged from 26～84 year with an average of 65 years.The duration of the disease varied from 1 week to l0 years.CT was done in 8,ultrasonography Doppler in 40,transesophagial echography(TEE)in 5 and angiography in 43.AAA reconstruction was performed in 40 patients,there was no operative death.TAAA was done in 4 patients,one died of cardiac accident during anesthesia.Three huge TAAA were sugically treated succesfully using improved shunting technique.The perioperative mortality rate in AAA was less than 5%,in TAAA was 25%.
