肝脏损伤的外科治疗

Surgical treatment of liver injury

目的总结156例肝脏外伤治疗的经验。方法1994年1月-2003年12月的156例肝脏外伤的外科治疗情况进行回顾性分析。结果本组病例采取综合手术治疗121例,治愈108例,治愈率为89.3%;死亡13例,死亡率10.7%,死亡原因为严重复合伤及严重休克。非手术治疗35例,治愈率100%。结论采用综合手术治疗可明显提高治愈率和减少并发症。肝修补术、清创加大网膜填塞修补术、不规则肝叶切除术、肝周纱布填塞法及选择性肝动脉结扎术是治疗外伤性肝破裂的主要手段。

Objective To summarize the treatment experience of 156 cases of liver injures. Methods The data of surgical treatment 156 cases of liver injure in our hospital from Jan. 1994toDec. 2003 was analyzed retrospectively. Results Among 121 patients of comprehensive surgical treatment, 108 cases were cured; the outed rate was 89.3%. 13 cases were dead, who died from serious associated injury and serious shock, the dead rate was 10.7 %. 35 patients of non - surgical management were cured totally. Conclusion The compehensive surgical treatment could improve the cured rate and reduce the complicated rate.The major means for wearing traumatic rupture of the liver were hepatic repair, debridment filling with cmentum and repair, irregular hepatectomy filling with fustian around the liver and selective ligafion of hepatic artery.
