摘要 目的探讨膜式液基薄层细胞学技术(Thin prepcytology test,TCT),在浆膜腔积液细胞学诊断中的价值。方法250例浆膜腔积液标本采用传统普通细胞涂片法为A组,240例浆膜腔积液标本采用液基细胞学检查为B组,将两组结果进行比较,数据采用卡方检验。结果采用TCT技术制片与传统涂片相比细胞多且集中,薄层均匀,背景干净。A组250例确诊癌30例(12%),其中类型难定4例(13.3%),可疑癌50例(20%),阴性170例(68%)。B组240例确诊癌58例(24.2%),其中类型难定仅2例(3.45%),可疑癌25例(10.4%),阴性157例(65.4%)。两者相比差异显著(P<0.01)。结论采用TCT技术检测浆膜腔积液可获高质量涂片,能大大提高恶性细胞诊断率,且癌细胞分类更容易,更能为临床治疗提供可靠的诊断依据。 Objective To study the value of Thinprep cytology test(TCT) in cytological diagnosis of serous cavity effusion(SCE). Methods 490 cases of SCE were divided into 2 groups that group A(250 cases) with routine smear test, group B(240 cases) with TCT. Analysis the difference of the diagnostic accuracy rate of malignant tumor cells) with TCT. Analysis the difference of the diagnostic accuracy rate of malignant tumor cells and quality of the smears between 2 groups. The data was managed statistically. Results Compared with group A, the structure of cells was distince, the cells were plenty and thin layer, and the background was clean in the thinprep slides. 30 cases of cancer were definite diagnosis( 12 96 ) and 50 cases were suspected of cancer(20 96 ) in Group A. 58 cases of cancer were definite diagnosis(24.2 96 ), 25 cases were suspected of cancer(10.4 96 ) in Group B. There were significant difference between two groups( P 〈 0.01 ). Conclusion Thinprep cytology test use in SCE, the smears are higher quality, the diagaostic accuracy rate of malignant cells is greatly increased, and can be more useful for clinic treatment.
