摘要 目的联合检测HIF-1α和Bak在乳腺癌中的表达情况,研究两者在乳腺癌发生、发展中的作用。方法应用免疫组织化学的方法观察50例浸润性乳腺癌1、2例非浸润性乳腺癌1、5例不典型增生乳腺组织1、5例正常乳腺组织腊块切片中HIF-1α和Bak的表达,并进行统计分析。结果在正常乳腺组织、不典型乳腺增生组织、非浸润性乳腺癌、浸润性乳腺癌中HIF-1α的阳性率为0、0、50.0%、58.0%,Bak的阳性率为20.0%、73.3%、83.3%、66.0%,两者表达在不同的乳腺组织间有显著性差异(P<0.01)。HIF-1α的表达与乳腺癌淋巴结转移和组织学分级密切相关(P<0.01)。Bak的表达与乳腺癌组织学分级有关(P<0.05),而与乳腺癌淋巴结转移无关。两者与患者的年龄、肿瘤的大小、临床分期无密切关系。在乳腺癌中HIF-1α和Bak的阳性表达之间呈相关性(P<0.05),但关系不甚密切(Pearson列联系数P=0.296)。结论HIF-1α和Bak基因共同参与了乳腺癌的演进、转移,可能成为预测乳腺癌侵袭、转移、预后的重要指标和在基因水平上治疗和预防乳腺癌的重要靶点。 Objective To explore the clinical significance, the interaction of HIF-1α and Bak expression in the course of carcinogenesis and progress of breast cancer. Methods Fifty women with invasive breast carcinoma were studied for the expression of HIF-1α and Bak. The expression of HIF- 1α and Bak in twelve cases with non-invasive breast cancers, fifteen eases with atypical hyperplasia breast tissues and fifteen cases with normal breast tissues were also examined by immunohistochemical assay. Results The positive rates of HIF-1α expression in normal breast tissues, atypical hyperplasia breast tissues, non-invasive breast cancers and invasive breast cancers were 0,0,50.0% and 58.0% respectively. The positive rates of Bak expression in normal breast tissues, atypical hyperplasia breast tissues, non-invasive breast cancers and invasive breast cancers were 20.0%, 73.3% ,83.3% and 66.0% respectively. Significant difference was observed in the expression of HIF-1α and Bak in different breast tissues ( P 〈 0.01) .The expression of HIF-1α was correlated with the metastasis of lymph node and histological grades of breast cancer( P 〈 0.01 ) . Statistical analysis showed that the positive expression of Bak was associated with histological grades of breast cancer( P 〈 0.05), and not associated with lymph node metastasis. The expressions of HIF-1α and Bak were not correlated with patients' age, tumor sizes and different clinical types. There was correlation between the expression of HIF-1α and Bak( P 〈 0.05) . But their relationship was not close (Pearson contingency coefficient P = 0.296) . Conclusions HIF-1α and Bak participated in the evolvement and metastasis of breast cancer. They may be the important indexes for forecasting the invasion, metastasis and prognosis of breast cancer and also may be the important targets for treating and preventing breast cancer in gene level.
