原发性颅内淋巴瘤的MRI诊断

MRI diagnosis of primary intracranial lymphoma

目的探讨M R I对颅内原发性淋巴瘤的诊断价值。方法回顾性分析14例手术病理证实的颅内原发性淋巴瘤的M R I影像表现。结果多发12例,单发2例。位于侧脑室旁、基底节区及丘脑8例,胼胝体及前联合1例,脑室系统2例,枕额顶叶脑实质皮质区3例。肿瘤呈等T1等T2信号10例,呈稍长T1稍长T2信号4例。4例无明显水肿,9例肿瘤周围出现轻度水肿,位于脑室系统1例出现双侧脑白质弥漫性水肿,其中1例肿瘤出现坏死、囊变。增强扫描呈明显均匀团块状、结节状强化11例,环形强化1例,局部散在斑块状强化2例。结论颅内原发性淋巴瘤有典型的M R I特征,M R I对其诊断有较高的价值。

Objective To investigate the value of MRI in diagnosing the primary intracranial lymphoma. Methods The MRI findings of 14 cases primary intracranial lymphoma confirmed by histology were analysed retrospectively. Results 12 cases were multiple lesions and 2 cases single lesion. Lesions of 8 cases located white matter adjacent the lateral ventricle, basal ganglia and thalamencephalon. 1 case located corpus callosum and commissural rostralis. 2 cases located ventricle. 3 cases located the cortex of occipital lobe ,frontal lobe and parietal lobe. Lesions of 10 cases showed isointensity on T1WI and T2WI. 4 cases showed slightly hypointensity signal on T1WI and slightly hyperintensity signal on T2WI. Slight peritumoral edema occurred in 9 cases and did not in 4 cases. 1 case located ventricle occurred diffusing edema in the white matter cranial, cavity. 1 case showed cystic changes. Lesions of 11 cases showed markedly lumpish or tubercular enhancement, 1 case ring-like enhancement and 2 cases sheet enhancement in the local. Conclusion Primary intracranial lymphoma has the typical MRI feature. MRI can diagnosis them correctly.
