隐匿性阴茎的诊断与外科治疗(附40例报告)

目的通过对40例隐匿性阴茎患儿的诊断与治疗，了解外科手术对儿童隐匿性阴茎治疗的有效性。方法根据患儿不同病理特点，采用阴茎肉膜纤维索、耻骨前脂肪垫切除加阴茎根部皮肤固定术，重症患儿切断阴茎悬韧带浅层，对于单纯耻骨前脂肪肥厚患儿行包皮口纵切横缝、耻骨前脂肪垫切除及阴茎根部皮肤固定术。对于曾行包皮环切术的患儿行阴囊带蒂皮瓣转移术。结果40例患儿切口均一期愈合，无明显并发症，术后阴茎比术前延长2～3cm，2例复发患儿二次手术行阴茎根部皮肤固定后治愈，3例术后阴茎显露不够满意。有效率为92．5％。结论外科手术可有效治疗隐匿性阴茎，明显延长患儿阴茎的长度，同时改善了排尿症状和阴茎外观。

Objective To evaluate the effects of surgical treatment for concealed penis from 31 children cases. Methods According to the different pathological character of the patients, different operation styles were applied. To the patients with inelastic dartos fascia, the dartos and suprapubic fat pad were resected and dermedesis of root of the penis were applied. The superficial lameUa of suspensory ligament of the penis must be cut in the severe patients. To the patients with suprapubie fat pad thickening, the orifices of the prepuces were silted and sutured transversely. The dartos resection were not applied. Transfer of pedicle skin flap of serotum was applied to the patients after circumcision. Results Incisions of all 31patients were primary healing. No notable complications were found. The length of the penises was extended about 2 - 3cm. 2 recurrent cases received the second operation of dermodesis of root of the penis and were cured. The elongation of penises of 3 patients was not satisfactory. The total rate of effect was 92.5 %. Conclusion The surgical intervention can effectively treat concealed penis and notably extend the length. At the same time, the operation releases the symptom of mictien and improves shape of the penis.
