Survivin在儿童急性白血病中的定位表达及意义

Expression of survivin and its location in cell and clinical significance in pediatric acute leukemia

目的探讨Survivin在儿童急性白血病中的表达、细胞中的定位及与临床疗效的关系。方法应用免疫组织化学链霉亲和素生物素过氧化物酶复合物(SABC)方法,检测62例急性白血病患儿骨髓细胞Survivin的表达,Survivin在细胞中的定位,对照组为40例同期住院发热患儿骨髓(排除恶性血液病)。结果在62例急性白血病患儿中,Survivin蛋白表达阳性率为41.94%(26/62),明显高于对照组骨髓5%(2/40)(P<0.01)。Survivin在胞浆中的表达为46.15%(12/26),在胞核中的表达为53.85%(14/26),差异无统计学意义(P>0.05),但二者4周末完全缓解率差异有统计学意义(P<0.05),Survivin胞核表达阳性者完全缓解率高于胞浆阳性表达者。Survivin在高危、中危和低危急性淋巴细胞白血病中的表达分别为66.67%、50%、22.22%,差异无统计学意义(P>0.05)。结论(1)Survivin在儿童急性白血病中表达率为41.94%,提示其在儿童急性白血病发生、发展和预后中的意义。(2)Survivin在胞浆和胞核的不同表达,与临床化疗疗效有关,可能作为判断预后的一个指标。

Objective To investigate the expression of Survivin, its location in cell and clinical significance in pediatric acute leukemia. Methods The expression of Survivin protein was detected by immunohistochemical assay in 62 children of acute leukemia. The control was 40 fever children but with normal bone marrow during the same period. Results Survivin protein expression rate was 41.94% in 62 children with acute leukemia. It was significance higher than that in control group （P 〈0.01 ）. The expression rate of Survivin was 46.15% in cytoplasm and 53.85% in nucleus, the difference was not significant（P 〉 0.05 ） , but the rate of complete remission was significantly different in the end of four weeks （ P 〈 0.05 ）. The rate of complete remission in nucleus was higher than that in cytoplasm. Survivin protein expression were 66.67%, 50%, 22.22% in high risk, middle risk and low risk of acute lymphocytic leukemia. They were not significant difference（ P 〉 0.05 ）. Conclusion （ 1 ） Survivin protein expression rate was 41.94% in pediatric acute leukemia. It would play an important role in the development and prognosis of pediatric acute leukemia. （2） The different expression of Survivin in cytoplasm and nucleus would be related with clinic therapeutic effect. It may be a marker of prognosis.
