摘要
目的:探讨MRI和MRA对Wallenberg综合征的诊断价值。方法:回顾临床确诊的Wallenberg综合征患者20例,均行MRI,其中15例行MRA。结果:MRI检查显示,18例患者有延髓梗死灶,其中仅5例病变局限于延髓,13例同时存在桥脑、小脑或枕叶病变;另2例病变仅见于桥脑或小脑。MRA示病变侧椎动脉狭窄、闭塞、夹层分离。结论:MRI与MRA结合应用为Wallenberg综合征的诊断提供了梗死灶及责任血管判断的依据。
Objective: The diagnostic significance of MRI and MRA in patients with Wallenberg syndrome was evaluated. Methods: Twenty adult patients diagnosed to have Wallenberg syndrome based on the clinical manifestations were analyzed. MRI was also performed on all the patients, and magnetic resonance angiography was carried out on 15 patients. Resultst Out of the 20 patients, 18 showed a lateral medullary infarction corresponding to clinical defects on MRI images, and 2 patients with the infarction beyond the medulla. In detail, 5 patients presented a lateral medulla infarct alone, 13 patients with an infarct involving the medulla and pons or the ipsilateral cerebella or oe cipital lobe, 1 patients with a pontine infarction alone, and 1 patients with the cerebellar infarction alone. Out of the 15 patients on whom the MRA was executed, occlusions or dissecting or stenosis in the ipsilateral vertebral artery were shown. Conclusion: The combined use of MRI and MRA provides a reliable diagnostic method for illustrating the infarction site and corresponding vessels in patients with Wallenberg syndrome.
