纤维乳管内视镜在176例乳头溢液中的应用体会

Experience of the use of breast fibieroptic ductoscopy in 176 patients with abnormal nipple discharge

为了探讨乳管内视镜在乳头溢液中的临床意义,用纤维乳管内视镜对176例乳头溢液患者进行检查,观察溢液导管及其分枝的管腔和管壁结构,发现并确定病灶位置,记录病变导管形态特征,必要时用活检针取组织送病检,对资料进行回顾性分析。176例患者中,其检查190孔,导管内乳头状瘤75例(42·6%),导管内乳头状瘤病31例(17·6%),导管内乳头状癌11例(6·3%),慢性炎症46例(26·1%)及导管扩张症9例(5·1%%),4例未发现异常(2·2%)。初步研究结果提示,乳管内视镜可明确引起乳头溢液的病因,确定病变部位,从而提高早期乳腺癌的发现率。

The objective of this study was to investigate the clinial significance of fiberoptic ductoscopy in patients with nipple discharge. A total of 176 patients with nipple discharge were examined by fiberoptic ductoscopy to investigate the concerned duct and its branch and define the location and extent of intraductal leisions. Core biopsy was applied in suspicions leisions. All the 176 patients were inserted successfully into 190 orifices, and papillomas was found in 75 cases （42.6 % ）, papillomatosis in 31 cases （17.6 % ）, intraductal cancer in 11 cases （6.3 % ） ,mammary duct ectasia in 9 cases （5.1%） and galactophoritis in 46 cases（26.1%）, and 4 cases were normal（2.2%）. In conclusion, fiberoptic ductoscopy can locate the accurate site and define the pathogeny of nipple discharge, and improve the diagnosis of early breast cancer.
