前后路一期手术治疗颈椎骨折脱位

One-stage anterior and posterior operation for the treatment of cervical fracture and dislocation

目的 探讨颈椎骨折脱位前后路一期手术治疗的疗效及风险。方法 31例颈椎骨折脱位患者施行前后路一期手术复位内固定治疗。记录每例患者手术时间、术中出血量等指标，并于术后对患者神经恢复情况进行随访。结果 31例随访时间5～68个月。平均手术时间（125±21）min；平均出血量（82±28）ml；术后31例均获完全复位。无伤口感染、呼吸衰竭等并发症发生。15例患者神经功能有明显恢复。结论 该术式使受压脊髓获得早期而彻底的减压，同时重建了脊柱稳定性，减少了术后并发症，是治疗颈椎骨折脱位积极有效的手术方法。

Objective To explore the effects and risks of one-stage anterior and posterior operation for the treatment of cervical fracture and dislocation. Methods 31 patients with cervical fracture and dislocation were treated with one-stage anterior and posterior operation. The operation time and intraoperative blood loss were recorded . The neural function recovery was followed up. Results All cases were followed up for 5 - 68 months. The mean operation time was （125 ±21 ） min , mean blood loss （82 ±28 ） ml. All cases gain completely reduction of the spine. No wound infection or respiratory failure occurred. 15 cases got significant neural function recovery. Conclusions Onestage anterior and posterior operation for the cervical fracture and dislocation has advantages of early and completely decompression to the spinal cord, reconstruction of the spinal stability and diminishing the complication.
