532nm激光治疗视网膜大动脉瘤

Photocoagulation on Large Retinal Aneurysm by 532nm Laser

目的 观察532nm激光光凝治疗视网膜大动脉瘤的临床疗效。方法 依据荧光眼底血管造影检查确诊视网膜大动脉瘤22例22只眼，应用532nm激光，以直径100～150μm小光斑直接对瘤体光凝，对瘤体周围视网膜采用直径200～400μm光斑进行光凝，覆盖瘤体周围。曝光时间0．1～0．5s，功率0．3～0．9W，激光反应级别2～3级对光凝治疗效果进行随访观察。结果 随访5～24个月，视力增加者18只眼（82％），其中增加2行以上者8只眼；视力不增加者4只眼。随访期荧光眼底血管造影检查12只眼，见瘤体萎缩、出血、渗出逐渐吸收，视力不增加者合并有黄斑出血、视网膜前或下出血及屈光间质混浊。结论 532nm激光直接对视网膜大动脉瘤瘤体光凝，促瘤体闭塞以减少反复出血，缩短病程，有利于病情的恢复。

Objective To observe the therapeutic effect of 532 nm laser on large retinal aneurysm. Methods Twenty two patients （22 eyes） with large retinal aneurysm were proved by fundus fluorescein angiography （ FFA ） and treated by 532 nm laser photocoagulator with 100-150μm photocoagulation areas of aneurysm directly, and 200- 400 μm photocoagulation areas around the aneurysm with irradiation time 0. 1- 0. 5 s, power 0. 3- 0. 9 W, and 2-3 grades of spot reaction. Follow up time was 5- 24 months after photocoagulation, and FFA was performed on 12 eyes. Results In 18 （82%） eyes central vision was increased after photocoagulation. At follow-up time, the neoplasia body was atrophied and the bleeding and exudation were absorbed gradually. Conclusions The therapeutic photocoagulation using 532 nm laser is a better method for emphraxis of neoplasia body and shortening the cowrse of disease.
