口外弓在安氏Ⅱ~1中度拥挤错矫治中的应用

Application of Facebow in Orthopedic Treatment of Angle II^1 Malocclusion with Moderate Crowding

目的探讨口外弓在安氏Ⅱ1中度拥挤错牙合矫治中的应用。方法选择处于生长发育期的安氏Ⅱ1上颌牙性前突伴下颌轻度后缩的中度拥挤错牙合患者17例,先应用口外弓远中推移上颌第1恒磨牙,在上牙弓开拓间隙,再配合固定矫治技术排齐牙齿,解除拥挤,改善突度,矫正深覆牙合和深覆盖。结果17个病例总计34个磨牙向远中移动。所有上第1恒磨牙在口外弓的作用下均明显向远中移动,最多达5 mm,最少3 mm,平均3.5 mm;平均疗程5个月,磨牙远中移动的速率为1.5 mm/月。结论口外弓作为一种辅助装置,可以有效地远中移动磨牙,配合固定矫治技术可以加强口内支抗,为固定矫治技术解决牙列拥挤,上颌前突,深覆牙合和深覆盖作了必要的先期准备,但应注意选择使用的时机。

[Objective] To explore the application of facehow in orthopedic treatment of angle IIl malocclusion with moderate crowding. [Methods] 17 cases of Angle Ⅱ^1 malocclusion with moderate crowding of growing children were selected. Facehow was used to push the upper first molar; space expansion was made among the upper dental arch. Then, fixed orthopedic appliance was used to align teeth, eliminate the crowding and improve the profile and occlusion. [ Results] All the 17 cases acquired satisfactory results after ophodontic treatment. The average treatment period was 5 months with an orthopedic rate of 1.5 mm/month. [Conclusion] Facehow is an auxiliary device that can effectively move the molars; combined with fixed orthopedic appliance, it can strengthen the anchorage. The best opportunity should be adopted for the application of facehow.
